I think is is only going to be an issue for poorly maintained or
unmaintained extensions.

There are only three GNOME extensions I use and care about: Apps-Menu,
Window List, and Dash-to-Dock.  The first two are developed by the
GNOME folks so you can expect they will be updated when v 45 is
released.   I looked at Dash-to-Dock's git repo and they are already
adding the support for GNOME 45.

While I am not a developer, from reading the porting guide it does not
seem to be a huge lift to make the necessary changes.

The biggest issue seems to be needing to support mutiple versions of an
extension for pre and post v45.
